Tourist Visa requirements and application forms for Thailand

(visa required only if staying longer than two weeks)

1. A passport valid for at least six months. (Passport must be signed.)
2. Two completed visa application forms.
3. Two passport-size photo taken within the past six months.
4. $35.00 embassy consular fee. (Do not send separately -- included in total cost.)
5. A recent bank statement as evidence of adequeate finances
6. A Copy of travel ticket or itinerary.
7. For non U.S.A. citizens copy of Green Card or visa valid for U.S..
8. A completed Capitol Visa Services Service Request Form. (One form per request)
