Tourist Visa requirements and application forms for Russian Federation

1. A passport valid for at least six months from the date of departure from Russia. (Must be signed)
2. One completed visa application form (click here for online form). Click on "Registration" first and obtain a "DeclarantID"(User ID) and Password and proceed.ALL questions MUST be answered
3. Two passport-size photos taken within the past six months. DO NOT staple to application form.
4. $140.00 - $450.00 Russian embassy consular fee. (Do not send separately -- included in total cost.)
5. A copy of the visa authorization based on the confirmed accommodations for the entire visit to Russia from the hotel or tourist agency providing the accommodations. (The hotel or tourist agency must be registered with the Russian Ministry of Foreign Affairs.)
6. A copy of the hotel voucher issued by the hotel or tourist agency providing the accommodations.
7. Non-U.S. citizens: a copy of front and back of Green Card.
8. A completed Capitol Visa Services Service Request Form.
