1. A passport valid for at least six months. (Passport must be signed.)
2. Two copies of the front page(s) of the passport, bearing photo and vital information
3. Two completed visa application forms.
4. Two passport-size photos taken within the past six months. (We can reproduce at $6.00 each.)
5. Two copies of front two pages of passport
6. $100.00 embassy consular fee. (Do not send separately -- included in total cost.)
7. Copy of travel ticket or itinerary.
8. A completed Capitol Visa Services Service Request Form. (One form per request.)
