Tourist Visa requirements and application forms for Nigeria

1. A passport valid for at least six months. (Passport must be signed.)
2. One completed online application form((fill out application on line, submit application, print acknowledgement slip and send with the rest of documents to Capitol Visa), or paper application ($20 additional)
3. One passport-size photo taken within the past six months.
4. $112 embassy consular fee to be paid on line
5. On a separate sheet of paper give two references in the U.S. with Names,
   Addresses, Phone numbers, and email addresses
6. Copy of round trip travel ticket or itinerary showing return or onward travel
7. If staying with a friend, Invitation from Nigeria and item 8. If staying at a
   hotel, proof of hotel reservation (item 8 not required)
8. Copy of first five pages of the passport of the person who is inviting you.
9. A completed Capitol Visa Services Service Request Form. (One form per request.)
