Tourist Visa requirements and application forms for China

1. A passport valid for at least six months. (Passport must be signed.)
2. Two completed Chinese visa application forms (second form can be a copy). Note the following: a) if applicant is born in China, Hong Kong or Taiwan, he/she must write name in Chinese in item 1.3 on the application form
3. One passport-size photo taken within the past six months
4. $130.00-$160.00 embassy consular fee. (Do not send separately -- included in total cost.)
5. A copy of round trip ticket or itinerary and hotel reservation in China
6. If Applicant is applying for a double or mutiple entry visa, appplicant must write a letter addressed to the Embassy of China explaining the reason
7. A completed Capitol Visa Services Service Request Form.
