Brazil Tourist Visa
Tourist Visa requirements and application forms for Brazil
- A
passport valid for at least six months. (Passport must be
signed.)
- One
completed visa
application form. (Please Note: On question 21 you MUST fill out the comments portion of this question(for example
sight seeing, visiting friends, etc.) if applying for a
tourist visa
- One
passport-size photo taken within the past six months.
- $140.00
embassy consular fee. (Do not send separately -- included
in total cost.)
- Copy
of travel ticket or itinerary.
- a
legilble copy of the applicant's drivers license (photo
must be clear, enlarged copy will help) or a phone or utility
bill bearing the applicant's name and address.
- CONFERENCE ATTENDEES: A letter from the host in Brazil.
- A completed Capitol
Visa Services Service Request Form. (One form per request.)
IMPORTANT: If applicant is under age of 18, the following additional items are also required:
- A letter signed by father and mother giving permission for minor to travel and that they will provide the child with sufficient funds to cover all aspects of his/her trip. If minor is travelling with only one parent, a letter of consent from the other parent must be submitted. Letter must be notarized. For divorced parents, papers attesting to full custody must be presented. Additional fess will apply for visas for minors.
- For children between ages of 3 months and 6 years, a certificate of polio vaccination stating kind of dosage. Stamp must be certified by local Board of Health.
