Tourist Visa requirements and application forms for Bolivia

1. A passport valid for at least six months. (Passport must be signed.)
2. One completed visa application form . after you click go to the bottom of the page and from the box on the left side, select type of visa (tourist, business, etc.) and fill out form on line and print.
3. One passport-size photo taken within the past six months. (We can reproduce at $6.00 each off of an original photo.)
4. Copy of round trip flight itinerary or travel ticket.
5. Copy of the hotel reservation or invitation letter from bolivian friends or relatives indicating dates of visit and host address.
6. International Certificate of Vaccination for Yellow Fever.
7. Proof of financial resources in the form of a bank statement from the applicant or the applicant's legal guardian (if minor) or a copy of a credit card valid for the duration of the trip.
8. A completed Capitol Visa Services Service Request Form. (One form per request.)
