2. One completed visa application forms Page 1 AND Page 2
3. One passport-size photos taken within the past six months. (We can reproduce at $6.00 each off of an original photo.)
4. Copy of travel ticket or itinerary.
5. International Certificate of Vaccination for Yellow Fever.
6. A completed Capitol Visa Services Service Request Form. (One form per request.)
