1. A passport valid for at least six months. (Passport must be signed.)
2. One completed visa application form.
3. One passport-size photo taken within the past six months. (We can reproduce at $6.00 each.)
4. $50.00 or $100.00 embassy consular fee. (Do not send separately -- included in total cost.)
5. Copy of travel ticket or itinerary.
6. Copy of confirmed hotel reservations.
7. A completed Capitol Visa Services Service Request Form (One form per request.)
