Tourist Visa requirements and application forms for Belarus

1. A passport valid for at least six months. (Passport must be signed.)
2. One completed visa application form.
3. One passport-size photo taken within the past six months. (We can reproduce at $6.00 each off of an original photo.)
4. $131.00-$350.00 embassy consular fee. (Do not send separately -- included in total cost.)
5. Copy of travel ticket or itinerary.
6. a) If staying for less than 30 days, must give name, address and phone of host on application form. b) if staying for more than 30 days: Copy of a letter of invitation from host tourist company. (If invitation is from an individual provide An original (not a copy) invitation. Your relatives or friends should obtain invitation (“zaprashenne”) for you at the local passport and visa office of the Ministry of Interior and send it to you. In case of emergency (unexpected illness, death of the relative, etc.) visas can be issued upon submitting a certified cable from a doctor or a clinic in Belarus.
7. A completed Capitol Visa Services Service Request Form (One form per request.)
