Tourist Visa requirements and application forms for Afganistan

1. A passport valid for at least six months. (Passport must be signed.)
2. One completed visa application form.
3. One passport-size photo taken within the past six months. (We can reproduce at $6.00 each off of an original photo.)
4. $70.00-$180.00 embassy consular fee. (Do not send separately -- included in total cost.)
5. a copy of the front two pages of your passport (pages bearing your biodata and signature).
6. If you are a non-US citizen, a copy of your Alien Resident card (green card), or a copy of your US visa.
7. Copy of travel ticket or itinerary.
8. A completed Capitol Visa Services Service Request Form. (One form per request.
