HOW TO OBTAIN A SECOND U.S. PASSPORT
In order to obtain a second passport, please send Capitol Visa Services the following:
- Two
completed DS-82
passport application forms. The Passport Agency requires
that you fill out this form online click here. After you
have filled out the form online, print 2 copies of it and
sign both copies to be sent to Capitol Visa Services with
the rest of your documents. If for any reason you cannot
fill out the form online, click
here to print out the form and fill it out by hand. In this case you must fax a copy of this DS-82 form as
soon as possible after you have filled it out so that
we may generate a form for you and include this copy of
DS-82 in the materials you send to Capitol Visa (the charge
for this service will be $15 additional and processing will
be delayed). In your fax, please indicate when we should
expect to receive your application.
- Two
passport-size photographs taken within the past six months.
- Your
current U.S. passport that:
a) is not damaged (if damaged, see instructions for
"replace
mutilated passport"); and
b) was issued to you when you were at least 16 years old and was issued for the full term of 10 years
- A personal check or money order (NOT
COMPANY CHECK) for $135.00 made payable to Department of State
(Please note- This fee is paid directly to the U.S. Department of State
and does not include any Capitol Visa fees.)*
- A statement
addressed to the Passport Agency requesting a second passport
with reasons why you need a second passport
- A copy
of a travel ticket or itinerary issued by airline or travel
agent.
- A letter
of authorization.
- One
completed Capitol Visa Services Service Request Form.
* Please note that this fee includes a $60.00 expediting fee MUST BE paid to the passport office in order to process your passport in less than 45 working days.
