AGES 15 AND UNDER
REQUIREMENTS
FOR OBTAINING A NEW PASSPORT FOR A MINOR AGES 0-15
To obtain
a passport for the first time, or to renew an expired passport
for a minor ages 15 and under, an applicant must submit:
- Two completed DS-11
passport application forms. The Passport Agency requires that you
fill out this form online. After you have filled out the form online,
click "create form", make sure a bar code appears on the left side of
the form you have filled out, print 2 copies of it and sign one copy
before the clerk who executes the application form and follow all the
instructions below in the section "The
application for a new passport must be executed". If for any
reason you cannot fill out the form online, click
here to print out the form and fill it out by hand. In
this case you must fax a copy of this DS-11 form as soon as possible
after you have had the application executed so that we may generate
a form for you and include this copy of DS-11 in the materials you send
to Capitol Visa (the charge for this service will be $15 additional
and processing will be delayed).
- Two passport-size photos taken within the past six months.
- An ORIGINAL or CERTIFIED

(another check for $25.00 will be paid to the post office clerk or clerk

of $120.00 is paid directly to the US State Department and does not
include any Capitol Visa fees.)

letter of consent signed by the non-applying parent or
