Replace U.S. Passport - Damaged Passport

REQUIREMENTS FOR OBTAINING A NEW PASSPORT
TO REPLACE A MUTILATED PASSPORT
To obtain a passport as a replacement for a mutilated passport,
an applicant must submit:
- Two
completed DS-11
passport application forms. The
Passport Agency requires that you fill out this form online. After you have filled out the form online, click “create
form”, make sure a bar code appears on the left side of
the form you have filled out, print 2 copies of it and
sign one copy before the clerk who executes the application
form and follow all the instructions below in the section
"The application for a new passport must be executed".
If for any reason you cannot fill out the form online, click
here to print out the form and fill it out by hand. In this case you must fax a copy of this DS-11 form
as soon as possible after you have had the application
executed so that we may generate a form for you and
include this copy of DS-11 in the materials you send to
Capitol Visa (the charge for this service will be $15
additional and processing will be delayed).
- One
completed DS-64
statement form. MAKE SURE to explain
on this form how your passport got mutilated.
- Two passport-size photos taken within the past six months.
- An ORIGINAL or CERTIFIED COPY of a U.S. birth certificate. (NOT hospital
record), or a previously issued passport, or an ORIGINAL Certificate of Naturalization.
- A personal check or money order.
(NOT company check) made payable
to Department of State for $170.00* and another
check for $25.00 will be paid to the post office clerk or clerk of court
who executes the application.) (Please note -- the above fee of $170.00
is paid directly to the U.S. State Department and does not include any
Capitol Visa fees.)
- A
copy of travel ticket or itinerary issued by airline or
travel agent.
- A letter of authorization.
(Signed by the applicant.)
- One
completed Capitol Visa Services Service Request Form.
This
application must be executed. This means
that items 1 through 7 above
must be taken by the applicant to a
clerk
of court or to a
designated
postal employee who is authorized to execute passport
applications at a local post office to be sealed in an
official envelope (you might wish to call your local post
office to find out which post office in your local area
is authorized to execute passport applications). This
sealed envelope
(DO NOT OPEN) plus
additional
copies of items 1, 6, 7 and a completed Service request
form(item 8) above must then be sent to Capitol Visa Services
in a second envelope outside the sealed envelop.
