American Passport Amendment
YOU CAN USE THIS FORM ONLY IF YOUR PASSPORT HAS BEEN
ISSUED LESS THAN ONE YEAR AGO.
HOW TO HAVE AN AMERICAN PASSPORT AMENDED
In order to have your passport amended, please send Capitol Visa Services the following:
- One DS-5504
passport application form competed on line. You may use this form
for one of the following reasons:
a) When there is a change in your identifying information,
such as a name- change due to marriage or by court order;
or
b) To Extend the validity of certain previously limited
passports; or
c) To correct a Passport Agency's printing error
- Your
present passport that needs to be ammended.
- Documentation
to support your amendment, i.e. birth certificate, marriage
license, court order (These must be original documents and
will be returned to you with your amended passport.)
- A personal
check or money order (NOT COMPANY CHECK)
for $60.00 made payable to Department of State(Please note
- This fee is paid directly to the State Department and does
not include any Capitol Visa fees.)
- A copy
of travel ticket or itinerary issued by airline or travel
agent.
- A letter
of authorization. (Signed by the applicant.)
- One
completed Capitol
