Passport Application - Additional Pages
HOW TO ADD PAGES TO A U.S. PASSPORT
In order
to have your passport amended, please send Capitol Visa Services
the following applications/forms:
- One DS-4085
"Application for Additional Pages" Form
completed on line. Make sure to fill out on the form sections for mailing
address and permanent address both even if they are the same.
- Your
present passport.
- A personal
check or money order (NOT COMPANY CHECK)
for $60.00 made payable to Department of State. (Please note
-- This fee is paid directly to the State Department and
does not include any Capitol Visa fees.)
- A copy
of travel ticket or itinerary issued by airline or travel
agent.
- A letter of authorization (Signed
by the applicant).
- One completed Capitol Visa Services Service Request Form.
